Athletic Forms: Alliance City School students in grades 7-12, who plan on participating in any sports for the 2016-17 school year, MUST have a current physical and insurance packet on file. These forms can be turned in to their coach or the athletic office. Fall sport athletes must have these turned in no later than August 1st. Please be sure to take the current 2016-17 physical form with you to your doctor's appointment. This form and the insurance packet can be downloaded from the high school website or found outside the high school ticket office or the middle school office.
